This office accepts payment in the form of cash, credit cards and checks with a valid Florida Drivers License or Valid Florida Identification Card for Rheumatology services which apply to Ocean Ridge Arthritis Associates, P.A. Dr. Starkman participates with many local insurance plans but copayments, coinsurances and deductables which are patient responsibility are due at the time of service. We appreciate your cooperation with this practice policy.
With respect to Medicare patients, we do not bill secondary insurances.Therefore if your insurance secondary to Medicare is not a medigap or automatic crossover to medicare, please be prepared to pay your portion of your bill at the time of service.
This office DOES NOT ACCEPT insurance or credit card payments for acupuncture services provided by Advanced Acupuncture Alternatives, P.A. Payment for acupuncture services include cash or checks with a valid Florida Drivers License or Valid Florida Identification Card and are due at the time of service.
